To start using Fillable Claims Form, you need to first download the
install the ACORD Viewer linked below. If you need assistance in setting
up the program on your computer, please contact Ron Bridge at

Once the ACORD Viewer is downloaded and installed, you can use the
Fillable form. This option allows you to email, print, save or fax the
form.
• Fill, Save and Send - Now you can fill out each form on screen and

• On-Screen Help - Every field features on-screen help and prompts to
make filling out the forms easier.
• Forms Instruction Guide - An “Instructions” link brings up the full
instruction guide for that form, right on screen.
IE: "Right Click" on the link "Save Target As" onto your desktop, then
open with ACORD viewer.
